脳神経外科医への道 Vol. 56

 当科の初期研修医、古賀悠一郎先生が執筆した論文がSurgical Neurology International誌に掲載されました。もやもや病に罹患した小児が四肢の不随意運動で発症することはこれまでにも報告されていましたが、今回報告した2例は顎口腔ジストニアと呼ばれる不随意運動で発症しました。このきわめて稀な神経徴候は大脳基底核および頭頂葉の脳虚血が原因と考えられ、STA-MCA anastomosis + EDMAPS術によって完全に消失しました。
 古賀先生は医学部6年生の時も当科で論文を書いて、Neurologica Medico-Chirurgica (Tokyo)誌に掲載されており、将来が大変楽しみな男です。

A new article written by Dr. Yuichiro Koga, one of residents at our department, has been published in Surgical Neurology International journal. Involuntary movements of the extremities in children with moyamoya disease have been reported in the past, but the two cases reported here presented with involuntary movements called oro-mandibular dystonia. This extremely rare neurological symptom is considered to be caused by cerebral ischemia in the basal ganglia and parietal lobes, and completely disappeared after STA-MCA anastomosis + EDMAPS surgery.
 Dr. Koga also wrote a paper at our department when he was a 6th grade medical student, which was published in Neurologia Medico-Chirurgica (Tokyo) journal, and I am very much looking forward to his future.
