もやもや病に対する直接的バイパス術はきわめて有効な治療法ですが、まれならず一時的に血流が流れすぎてしまう過灌流現象に遭遇します。この現象を見落とすと重篤な合併症を招くこともあるので注意が必要です。最近、この現象に関するデータをStroke誌に発表しましたので、興味のある方はご一読下さい。
Direct bypass surgery can rapidly improve cerebral hemodynamics and is a quite useful technique for patients with moyamoya disease. But, it should be reminded that postoperative hyperperfusion can often be observed after surgery. Critical perioperative management would be essential to avoid serious complications.
